Program-Specific Instructions

IMPORTANT

In addition to the above, applicants are required to submit a DNP Departmental Form (Prospective Advisor Form) signed by a prospective advisor who has agreed to serve as your primary advisor upon admittance into a degree seeking graduate program.

When you identify an advisor of interest, it is recommended that you email the individual and include your research interests and your resume. If they agree to advise you, they will need to complete the DNP Departmental Form (Prospective Advisor Form) and return it to you. Submit the form as part of your application materials in the supplemental documents section under "additional/optional forms".

Click here for a list of faculty that has indicated a desire to accept qualified students who align with their research interest and are open to consider serving as a primary advisor/committee chair.

Applications submitted without the DPN Departmental Form will not be processed.
